Register to Become a Member

To become a web member or request more information, please fill out the easy form below (and be sure to select "Register to Site" in the Category). When writing to us, please specify the following:
  1. Are you actively seeking biological family? (Yes or No)
  2. Are you? (Please choose one)
    • Adoptee
    • Birth Mother
    • Birth Father
    • Birth Sibling
    • Other (aunt, uncle, adoptive family)
  3. Please tell us about yourself and your search. All information is given in the strictest of confidence and is only read by FMNFS VP Marnie Tetz. Web Membership does not include our quarterly newsletter, yet all other services and support are here for the asking. (This is required for Membership).
  4. Select "Register to Site" from the Category box.


Contacting a Member

To contact a member of The Forget Me Not Family Society, please include their name in the subject line when sending to the Society's email address. Please select "Contact Member" from the Category list.


Register to Receive Newsletter

To become a paid member of the Forget Me Not Family Society ~ Adoption Circles, and to receive our quarterly newsletter, please print out the form, complete it and sent it with payment to the address provided. The cost for an Individual Membership is $25.00 annually and Corporate Membership is $60.00 annually and receipts will be issued.
